Customize keyboard shortcuts
Added a new menu item "Environment -> Customize keyboard..." that opens a
file with all shortcuts in the program. Just edit the shortcuts you want to
change and save (and restart the program).

Fold regions (C#, PHP, ...)
Some languages, like C#, use #region ... #endregion to define a foldable region.

This should now work properly in both C# and PHP (and other languages if defined
in the syntax file).

* Load file issues (display correct encoding in the statusbar).
